See attached employment contract to be included on the signature page if the user chooses Supervision Direction or Control. Can this please be displayed on the signature page and made visible from an expansion dropdown button. When the contract is signed I would like the signature to be at the bottom of the contract. See attached Self Employed Terms and conditions to be included on the signature page if the user chooses NO supervision Direction or Control. Can this please be displayed on the signature page and made visible from an expansion dropdown button. When the contract is signed I would like the signature to be at the bottom of the contract. Feel free to amend the layout.

See attached Self Employed questionnaire to be added to the registration form upon the user choosing NO supervision Direction or Control. A simple Yes/No Radial button will suffice for these questions.

See attached the expenses policy to be accessible via link on the expenses form where people have ticked the box to indicate they have already read the policy.
